"Modern Family" actress Sarah Hyland and Matthew Prokop ended their 6 year relationship late last month and has since been granted a temporary restraining order from her ex-boyfriend.

"On Sept. 19, 2014, Ms. Hyland obtained a Domestic Violence Temporary Restraining Order against Matthew Prokop," said Hyland's lawyer Lee A. Sherman of CTSC Law in a statement to The Huffington Post

"The documents filed speak for themselves. Out of respect for the court, the process and all parties, I have advised Ms. Hyland not to comment on the matter. We request that you respect the parties' privacy during this time," concluded the attorney.

According to documents accessed by TMZ, Hyland, 23, claims that Prokop, 24, "choked her, pushed her and threatened her life ... and her TV mom witnessed some of the abuse."

The docs reveal a particularly disturbing incident in May when an argument regarding Hyland's outfit turned physical as Prokop allegedly pinned the actress against a vehicle and proceeded to verbally chastise her--calling her a "c**t" repeatedly and choking her.

"His grip was so tight that I could not breathe or speak. I was scared and in fear for my life," relayed TMZ based on the documents. The event gave the petite actress a "very sore throat."

The documents also outline how Hyland wanted out of the abusive relationship and sought the help of her on-screen mother, actress Julie Bowen, 44, to intervene and assist her in ending the relationship peacefully. Apparently, upon seeing Bowen, Prokop reportedly freaked out and "ran outside into the backyard and began screaming."

It was during this time that Hyland was told by Bowen to leave the home as she believed Prokop posed a danger to her TV daughter. Bowen also alleges in the documents obtained by TMZ, that Prokop has threatened to set one of her homes on fire and harm her pet dog.

Further, the nightmare behavior didn't cease when the relationship ended. Prokop has "relentlessly bombarded [her] with vile, threatening and emotionally disturbing texts and voice mails including his own suicide threats," said the actress who plays Haley Dunphy.

The documents also show that Prokop was in a rehab facility but was released Sept. 21. The director of the facility contacted Hyland and advised her to get the said restraining order against her former flame.

An LA judge has granted the actress a temporary restraining order that requires Prokop to stay at least 100 yards away.

The pair met during their audition for "High School Musical" and co-starred in the Disney TV movie, "Geek Charming."
